How to Register | Registration Form | Cancellation Policy | Sibling Discount

How To Register:

1. Choose the program(s) that your child/children will be attending.
2. Complete the registration form.
3. Be sure to state whether or not your child will take the OPTIONAL LUNCH PROGRAM. Also, please indicate whether your child is attending as a PLAYER or a GOALIE.

   There is a sibling discount of 10% for every class and camp. Take 10% off the price of the camp before adding GST.

4. Carefully read all CPHC Policies and Waiver before signing. PLEASE NOTE THAT THE REGISTRATION FORM IS NOT VALID WITHOUT A SIGNED WAIVER.
5. Send in the registration form with payment. We accept Cheques, Visa or Mastercard. Forms can be faxed into our office at 416-322-3383 or mailed in to our office or dropped off in person. WE DO NOT ACCEPT REGISTRATION OVER THE PHONE.
6. A written confirmation letter will be mailed out within 2 weeks of receiving your form. If you do not receive a confirmation letter within 2 weeks, please contact our office.
